Medically Tailored Meals Survey for Healthcare Professionals

Survey Purpose

The California Food is Medicine Coalition (CalFIMC) is a network of community-based, nonprofit, medically tailored meal providers. We have developed this survey to help us understand how we can better partner with physicians and health plans to offer medically tailored meal services to their patients.

The survey should take about 5 minutes to complete.
