STONEGATE COMMUNITY HEALTH CHAMPION AWARDS |
Stonegate Community Health Centre is a charitable organization that provides health care services, health promotion programs, and community support services in the Stonegate Queensway neighbourhood in South Etobicoke. Our mission is to work “with the community to improve its well-being.” Stonegate prioritizes services and programs to isolated seniors, people living in poverty, immigrants and refugees, women, and those with barriers to accessing health care services.
As we prepare for our Annual General Meeting, we are excited to launch the Health Champion Awards-an opportunity to recognize individuals, groups, and organizations that have made a meaningful impact in our local community. We invite you to submit nominations for those who have gone above and beyond in contributing to the health, wellbeing, and vitality of our community.
We will be announcing award winners at the Stonegate’s Annual General Meeting in September, 2025. More information to follow (www.stonegatechc.org).

NOMINATION CRITERIA
Nominees may include individuals, groups, or organizations that:
• Demonstrate a strong commitment to the growth and improvement of their community;
• Show care and compassion for fellow residents and actively work to make the community a better place to live;
• Go above and beyond to give back, especially by supporting the most vulnerable members of the community;
• Contribute meaningfully to enhancing health, wellbeing, and addressing emerging local needs;
• Take initiative and demonstrate leadership through their professional or volunteer efforts to improve their community;
• Exemplify the power of collective action and what can be achieved when people work together for the common good.
Nominees can be nominated for more than one of the following four categories:

To ensure your nominee is considered for the Community Health Champion Award, please complete the nomination form in full. While not required, we encourage you to include a photo of the nominee as part of your submission package.
Important Notes:
• Incomplete nomination packages will not be considered.
• Only successful nominees will be notified.
• All submissions become the property of Stonegate CHC and will not be returned.
• Members of the Stonegate Health Champion Award selection committee are not eligible to submit nominations.
