Harney County Community Health Needs Assessment 2019

Community Survey

We need your help to understand the health and health needs of people living in the Harney County community. The results of this survey will inform the community and its leaders of what health issues Harney County residents face, and what they consider important in improving health, wellness and access to needed health care. The survey is a joint collaboration of Harney County Health District, Harney County Health Department/High Country Health and Wellness Center and Symmetry Care, Inc.

There are 32 questions, and the estimated time to complete the survey is 10 minutes or less. To thank you for your time, we’d like to enter you in a drawing to win a $100 gift certificate to the Harney County business of your choice (three winners will be drawn). To enter, provide your contact information at the end of the survey. Your survey responses are completely confidential, and contact information will only be used for the purposes of the drawing.
