Student Assent

To help us learn more about you and your school, we are going to ask you to participate in a survey today and again in the spring and next year. The questions in the survey ask about you, your school, your friends and things that you may or may not have done. In a sense, many of your answers will count as "votes" about what you think goes on in your school. 

Here are some examples of the questions we will ask you to answer:

How much of a problem is students teasing, spreading rumors and lies, or saying mean things to other students? How wrong or okay is it when students ignore it when someone weaker is being pushed around? Do you think your teachers are fair? How often have you joined in when students told lies about other students? Students my age are there for me whenever I need help.

I want you to know that your participation in this survey is voluntary, which means that only you can decide if you want to participate. If you do decide to take the survey, you are free not to answer any question you don’t want to answer.

This survey is completely confidential, which means that no one will know what your answers are. We use a number, not your name, on the survey.  We will not and cannot show anyone your answers, not even your teacher, your principal, or your parents. It is extremely important to us that you trust us to keep what you say private. It is also important that you know this is not a test and what you tell us will not affect your grades. We just want to know what you think about things.
