  • Enrollment Application
  • Health Information Forms
  • Overview of School Policies
  • Safe School Form
  • Academic Records Release Form
    • Your child's application is not complete until full academic records are received from his or her current school.  This form must be filled out in the office.
  • Completion of Screening Test (for incoming Kindergarten students only)
  • Immunization Records
  • Birth Certificate
  • Medicaid card, if applicable
 
2 PROOFS OF RESIDENCY ARE REQUIRED:
One must be a bill-Electric * Gas * Water

Copy of Rental/Lease Agreement
Copy of Jackson County paid personal property tax receipt.

***These documents may be dropped off in the office.
