Thank you for taking the time to consider completing this survey.

Purpose: Your voluntary participation in this survey is being requested to help us better understand how geriatricians around the world assess and manage unintentional weight loss in older adults and their opinions around the prescribing of ice cream to address it. Interestingly, no one to our knowledge has examined geriatrician practices around these issues. The purpose of this study is to address this knowledge gap. This study has not received funding from commercial or pharmaceutical sources.

Survey Instructions: By entering the survey, you are consenting to participate. It should only take approximately  10 minutes. None of the survey questions can or will be used to identify you. You can discontinue completion of the survey at any time with no repercussions by simply exiting the link and closing your web browser.

Upon completion of the survey, you may enter your email address to be included in a random draw for one of five $100 USD Visa prepaid gift cards. 
 
Assurances: Information from individual surveys will NOT be released to anyone other than the study investigators. Your responses to the survey will be combined with the responses of others. Anonymized data from all of the surveys will be analyzed together by our research team.
 
Responsible Parties, Funding Sources, and Ethics Approval: This survey is being conducted by Dr. Samir K. Sinha, Director of Geriatrics at Sinai Health System and University Health Network in Toronto, Canada. This study has been funded entirely through the Sinai Health System Healthy Ageing and Geriatrics Program's research funds and has been approved by the Mount Sinai Hospital Research Ethics Board (MSH REB 19-0175-E).
