This form serves as the Facilitator Workshop registration along with the Work Exchange and Scholarship application.

As a grassroots nonprofit, Creative Strategies for Change (CSC) relies on volunteerism and community support to accomplish our mission of mobilizing arts and education for social justice.

If you've been enrolled by your organization to participate in this workshop, please be aware that every registered individual is required to complete this survey.

This pre-survey is required in order to complete registration for the Spring 2024 Facilitator Workshop. Your registration is not 100% complete until you submit this survey.

If you want to attend the workshop via Scholarship or Work Exchange to supplement the financial pressures of attending the workshop, you will be asked additional questions in this survey.

Scholarship - Grants you free attendance to the Creative Strategies for Change Facilitator Workshop. A 30 minute interview is required BIPOC ONLY

Work Exchange - Work 20 hours in the CSC office in exchange for attendance to the Creative Strategies for Change Facilitator Workshop. OPEN TO ALL
