Informed Consent Agreement
The PacWell survey will take you about 10-15 minutes. When you complete the survey, your name will be entered into a random raffle to win a gift certificate for the UOP bookstore.
This survey is confidential. You may discontinue at any time. Your participation will have no effect on your grades or your relationship with Pacific faculty or staff. Data that could be shared or published will be general responses from the survey without names or identifying information.
