Thank you for considering taking part in this questionnaire research study.

A lack of change to modifiable lifestyle factors such as diet and physical activity underpin a worldwide epidemic of non-communicable disease. In the UK, a call for primary care to take a leading role in tackling this epidemic has emerged. However, there is a lack of evidence and there remain opposing views about physical activity as a strategy for managing lifestyle related disease. This study has been designed to assess how previous and continuing training provides GPs with knowledge and skills for managing lifestyle related disease with physical activity interventions.

You have been invited to take part in this study because you are a practicing GP with experience of delivering lifestyle advice to patients, including advice around physical activity.

The questionnaire consists of 34 questions separated into parts related to knowledge and experience, assessment of physical activity, exercise prescription, and your own physical activity behaviours.

You will also be asked about your age and gender but no personally identifiable information will be recorded and all responses are anonymous. You will also be asked if you would like a summary of the findings from the study should you wish.

If you wish to complain about any aspect of the way in which you have been approached or treated during the course of this study, you should contact Dr David Nunan, (email david.nunan@phc.ox.ac.uk or telephone +44 (0)1865 617938).

Thank you for considering taking part.
