You are being asked to participate in a research study. This form provides you with information about the study. Please read the information below before deciding whether or not to take part. Your participation is entirely voluntary. You can refuse to participate or stop participating at any time without penalty or loss of benefits to which you are otherwise entitled. You can stop your participation at any time and your refusal will not impact current or future relationships with UT Austin. Purpose: We want to understand how people with visual impairments use health information.

Procedure: Up to 100 individuals with visual impairments will be asked to complete a series of questions. These include asking individuals to read an ice-cream container label and then answer 6 questions about it. They will be asked to complete a health knowledge questionnaire, and a background questionnaire as well as two questions on health and visual function. You may ask to complete the information over the phone, on-line, in Braille, or in person. Just contact the researcher for your preferred means and this will be accommodated. If you prefer, you may complete the form on line at this time.

Time: We expect reading and answering the questions will take about 15 minutes.

Risks/Benefits: The risk associated with this study is no greater than everyday life. There are no benefits for participating in this study.

Compensation: There is no compensation to you for completing these questionnaires.

Confidentiality: You will not be asked to give us your name, so your anonymity will be protected. The records of this study will be stored securely and kept confidential. Authorized persons from The University of Texas at Austin and members of the Institutional Review Board have the legal right to review your research records and will protect the confidentiality of those records to the extent permitted by law. All publications will exclude any information that will make it possible to identify you.
