FilmQuest 2026 Mandatory Survey

A survey of essential questions seeking information to help us plan and program the 2026 for the 13th edition of FilmQuest this upcoming October 22nd - 31st, including film information, filmmaker info, and other essentials. The answers provided will also help us program the virtual exclusives of the festival. Please fill out the survey as completely as possible by Wednesday, September 9th to help us schedule and plan for the festival efficiently.

This is a mandatory survey. By completing this survey you will be acknowledging and agreeing to having your film screen and/or screenplay compete at the 2026 FilmQuest without complication, and allowing the festival to promote your project(s) and the festival with your information.

We will be using Simple DCP for film uploads and delivery for the 2025 festival, which you'll receive the notice for in a separate email. We'll reach out to you directly if we don't get your film file from Simple DCP for whatever reason. High-Res .MOV and .MP4 files only. NO DCP's!!! (Note: 4k & surround sound files are accepted but please give us a stereo backup too, list N/A if your project is a screenplay. Subtitles need to be "baked" in to the film. No SRT files. For consistency, Apple Pro Res 422 are best. 1080p resolution and above. Please test files for errors before sending/uploading. DO NOT SEND US MASSIVE FILES.)(Required.)
Photo links to the promotional high-res screen grab photos from your film. Please send us no more than 3 to choose from. Do not include BTS photos. (Dropbox, Wetransfer, FilmFreeway, email, etc. links are all acceptable). You should be able to upload these via SimpleDCP, but if not you can still just use this form below. Do you plan on attending the physical festival (Oct. 22 - Oct. 31)? Please be as truthful as possible as this helps us schedule the festival. We highly recommend attending the festival for as many days as possible as its a can't miss event that creates serious FOMO for those who don't attend, but we understand if you or a representative from the film cannot. (Required.)
How many of your project's team, including yourself, do you anticipate attending the festival in-person?(Required.)
We will be collaborating with EO Flix, aka "EO," for the hybrid virtual portion of the festival where those cannot attend the festival can also participate and where their friends/family/crews can purchase virtual tickets and passes to the festival to watch all the participating films from afar. If you haven't yet, please create your profile and set up your project and link it to FilmQuest so you can screen via the following link and add festival director Jonathan Martin to your network to connect. Screening virtually on the platform is not mandatory but recommended.
We are partnered with the Downtown Provo Marriott Hotel for a discounted rate, which is located only a block from the festival venue at Velour. For convenience here is the direct link to the hotel's discounted booking page, and we recommend booking ASAP as rooms will be limited and will be the most convenient and luxurious way to rock FilmQuest.
To help filmmakers travel and attend the festival, would you be willing to do an Airbnb/Hotel/Housing share with other filmmakers? If yes, we'll put you all in touch via an email thread so you can coordinate your stays together. (Required.)
If attending, will you be staying at our partner hotel (Downtown Provo Marriott), an Airbnb, another hotel or location, etc.? Worth noting is it makes our partner hotel very happy when guests stay with them, but we understand if you have other plans or budgetary needs.(Required.)
