Dear Families,

At Coburg North Primary School, we recognise that physically and mentally healthy children learn better.

In partnership with the Coburg North Primary School Education Sub-Committee and Merri Health, our school is committed to improving health and wellbeing for students, staff and families. 

This survey will help the school understand the health and wellbeing topics that are of most concern to families, students and staff and will help us prioritise future work.  It will also help inform the CNPS School Council Strategic Plan which is looking at how our school supports and responds to mental health and wellbeing issues in our school community.

We would like to learn more about how you think we can work together to make our school a healthier place.  You are asked to answer the questions as honestly as possible as the responses will be used to make recommendations and create an action plan.  Your participation in this survey is voluntary and all responses will remain anonymous.  Results of this survey will be shared with the school community once compiled. 
 
This survey should take approximately 10-20 minutes to complete. 

This survey will refer to the terms ‘health’, ‘wellbeing’ and ‘mental health’. We have included definitions below to explain what we mean by these terms.

Health: Health is a state of complete physical, mental and social well-being and not merely the absence of disease or infirmity.[1]

Mental health: Mental health is defined as a state of well-being in which every individual realizes their own potential, can cope with the normal stresses of life, can work productively and fruitfully, and is able to make a contribution to their community.[2]

Wellbeing: Wellbeing is not just the absence of disease or illness. It is a complex combination of a person's physical, mental, emotional and social health factors. Wellbeing is strongly linked to happiness and life satisfaction. In short, wellbeing could be described as how you feel about yourself and your life.[3]

Examples of factors that can influence wellbeing include: positive relationships with others; having enough money; employment; regular exercise; nutritional diet; sense of purpose, meaning or belonging; spiritual or religious belief; living in a fair and democratic society.
