Survey for Families & Natural Caregivers

Dear Family Members:

To answer this survey you must be 19 years of age or older and take this survey in English.

This survey is intended for the family members of persons with intellectual and developmental disabilities. If you have more than one family member that fits this description, please choose only one for the purposes of this survey, and/or complete a survey for each individual.

The survey is part of a larger research project that is exploring:
  1. How spirituality might contribute to the flourishing of persons with intellectual and developmental disabilities; and
  2. How staff in community living agencies understand and enact their role in spiritual literacy and support of the people they serve.

The purpose of this survey is to understand family perspectives on this topic.

Your participation in the survey is completely voluntary and anonymous. If you do not wish to answer a question, feel free to leave it blank.

Responses will be gathered, summarized, and used for research, publication, and education purposes. Your participation in the survey indicates that you are willing to have us include your anonymous responses in summarized data. Please know that once you have completed and submitted the survey, there is no way to withdraw your responses as all survey data is anonymized.

If you are completing a hard copy and handing your completed survey in to a staff member, know that they have a signed confidentiality agreement stating that they will not share your responses or identity with others. They have committed to keeping your identity confidential.
Note: Questions about religion and spiritual practices could surface uncomfortable emotional responses or memories.
