Thank you for taking the time to complete this progress report. The information you are about to provide is compared to data collected at the beginning and end of the Grant cycle. Using this data, the Oregon State Legislature considers whether or not to fund the ODE Farm to CNP Grant program each biennium. Your accurate and thoughtful responses to these questions play a crucial role in that decision process, and largely determines the amount of funding, if any, you will have available to purchase Oregon foods and/or provide garden and nutrition education in the coming school years.

This progress report is mandatory, and required of all sponsors that participate in the ODE Farm to CNP Reimbursement Grant for Oregon Grown or Processed food. The survey should take less than 30 minutes to complete. It may be helpful for you to review your baseline data from the start of this biennium (for new sponsors), or your final report from last biennium (for returning sponsors). REMINDER: This report is due September 30, 2022. After this due date, you will not be able to submit future reimbursement claims or receive additional funding until this report is submitted.
