You recently received a service at Sonder. We would like to ask you some questions about your experience so that we can identify things that need to be improved in order to deliver better services. This survey is completely voluntary and any information provided will be confidential. The survey will take approximately 5 minutes of your time.

When answering the questions below, please think about your most recent interaction with Sonder. This can include services received for mental health, allied health, employment support or care coordination. Services may have been received at a physical Sonder location or co-located site or in the community.
 
The answers you provide will be collected and used in a way that you cannot be identified, such as in a report format. If at any time you do not feel comfortable answering a question, you are not obliged to do so.
