This survey is being carried out by NAVCA* (the National Association for Voluntary and Community Action) on behalf of the Health and Care Voluntary Sector Strategic Partners**.

We would like to find out about the extent to which groups of people with different needs are represented on local health structures. This information will form part of a report regarding representation of voluntary and community organisations and their beneficiaries on these health structures. This will be shared with the Department of Health, NHS England and the NHS Commissioning Board, as well as other Strategic Partners and the wider voluntary and community sector.

All information will be kept confidential, and we will not share your personal details outside of NAVCA without your consent.

This survey will close on the 21st November.

If you have any questions regarding this survey or any of the issues within it, please contact Ellie Munro on 0114 289 3984, or email ellie.munro@navca.org.uk.
