Thank you for enrolling in the RI Regional Extension Center (REC). To better determine your practice’s needs, please complete a Self-Assessment Survey. It should take about 20 minutes and be completed before attending your REC Orientation session. If your practice has more than one site, please complete one form for each enrolled site.

The Self-Assessment should be completed by a person who is knowledgeable about the practice. After you complete the assessment your REC Relationship Manager will contact you to discuss next steps.

Important! You may go back through the survey to edit your responses at any point before submitting it by revisiting the survey link in the "RI REC Enrollment Thank You" e-mail sent to you or by going to the Provider Enrollment page of DocEHRtalk.org. NOTE: Cookies MUST be enabled and saved on your computer, and you must return to the survey using the same computer and web browser. To complete multiple surveys, cookies must be erased after each completed survey. (More information on cookies.)
