Survey on if providers and patients speak different languages

The MOA is addressing the challenges for accurate communication when providers and patients speak different languages . The data gathered through this survey will assist the MOA in assessing need, and providing possible services for doctors to use in communicating with patients.  As we all know, language concordance between patient and provider is the most effective strategy to improve communication and health outcomes for limited English proficiency.  The MOA believes that health plans should consider providing incentives for primary care physicians to improve services in their practice that target patients who have low health literacy or limited English proficiency.  By partaking in this survey, it will help the MOA to demonstrate the need for these incentives.
.Do you ask your patients about language needs at intake? (Required.)
.Does your practice track patients language preferences in their medical records?(Required.)
.Provide an estimate of patients in your practice for whom English is a second language:
.What is your primary language? ⃝
.What additional languages do you speak at home other than English?
.Can you write in another language? If so, which one(s)?
.Can you deliver quality medical care in another language?
.Do you offer any of the following in your practice?
.What is your practice setting?
.Once the data has been collected, the MOA will offer a follow up for those who have interest. If you would like to receive a follow-up to this survey, please leave your name and email address.
