Welcome to the David Lawrence Community Service Award nominations page. If you are here to nominate a Kaiser Permanente employee for a David Lawrence Community Service Award (DLCSA), you have come to the right place.
The DLCSA are named in honor of Dr. David Lawrence, former CEO of Kaiser Permanente (KP) and lifelong advocate and champion of improving community health. The Awards recognize individuals and groups across the KP regions who champion outstanding community benefit activities and initiatives demonstrating extraordinary efforts to improve the health of our communities. These efforts may be completely voluntary, or may be related to a person’s job or a group’s function within KP. International volunteer service is also eligible for the award.

A few quick tips as you begin:
- Before you begin nominating someone, read through all the questions first. You may find that you will need to do a bit more research and/or information gathering about your prospective nominee to answer some of the questions.
- If you don't think you can finish your nomination in one sitting, please consider using the "Word document template" that is linked below for your convenience. You can save this Word version to your computer and when you are done, you can cut and paste your answers into the nomination form below.
- Self-nominations are fully acceptable.
- Group nominees should be comprised of KP employees.
- If you are not sure whether your region is still accepting nominations, please see the list below for start and end deadlines in each region.

- Nominations must be made by a KP employee or physician (including colleagues, supervisors, or others who can describe and speak to the impact of the nominee's work). Self-nominations are also welcome.
- Nominees must be full or part-time permanent employees of KP during the year in which they are nominated.
- Prior applicants can be re
