Davis Food Co-op Focus Group Application 2022

Thank you for your interest in being a part of the Davis Food Co-op's Focus Group. In order to be chosen to participate in these Focus Group interviews, and earn a $75 Co-op gift card, you must complete the following questionnaire by March 14th. Before you complete this questionnaire, please read the following carefully:

In order to be considered for the Davis Food Co-op's Focus Group, you must be available during the following dates and times:

- Saturday March 26th from 10am-11:30am
- Saturday March 26th from 1pm-2:30pm
- Sunday March 27th from 1pm-2:30pm

These Focus Groups will require that you have previous shopping experience at the Davis Food Co-op in order to give adequate feedback. You will be asked a variety of questions about your experiences at the Co-op as well as general questions about your shopping patterns. You must be comfortable speaking in-person to a group of around 12 people and willing to give thoughtful, insightful answers.

In order to best reflect the Davis community and customer base of the Davis Food Co-op, participants will be chosen based on the City of Davis' Census data as well as shopper demographics gathered through customer surveys. The following questionnaire will ask some basic demographic questions and it is important to answer these questions honestly to be considered for participation.

The 24 selected participants will receive a $75 gift card to the Davis Food Co-op upon completion of their Focus Group interview. Selected participants will be notified on March 15th via the provided email address and will have 48 hours to accept the invitation before alternate participants are selected.
