Why Self-Assessment

Hello and congratulations! You are about to take an important step towards improving your psychological health. This is the first of a series of questionnaires designed to help you become more aware of who you are and the direction you want your life to take. That awareness can help you direct your life the way you want it to go.
You are encouraged to respond to as many questions as you can. Whether you write a little or a lot, your responses can provide both you and your psychologist with information and insight about where you are right now on your journey. Becoming aware of where and who you are right now might even inspire you to choose a different path or find new ways to pursue an important goal you've held all along.

Your responses are not "hammered in stone"
. They are a snapshot. When you meet with Dr. Dawson, you may have other impressions.

It is understood you are sharing personal information. Your responses will be used primarily to inform the initial assessment of your psychological health status. You should know that your responses may help Dr. Dawson understand better what kind of strengths and challenges you bring to treatment, and why you have chosen psychological services right now.  
The second way your information may be used is in Dr. Dawson's research on the readiness of clients for psychotherapy. Any research conducted by Dawson Psychological Services using your responses will not mention any identifying information and will be pooled with others. These assessments are voluntary and you may opt out at any time without any reason. If you complete the questionnaire, but you don't want the information included in research, please let Dr. Dawson know using contact information below or by informing him in person.
Please answer the following questions to the best of your knowledge and ability. The assessment is designed to take around 20 minutes, but you may take a shorter or longer time to complete it.
