Dear Participant, 

You are being asked to participate in a survey designed to assess the impact of new Chronic Thromboembolic Pulmonary Hypertension (CTEPH) Guidelines on the diagnosis and management of the disease. The survey will take you through a series of questions to assess your knowledge pertaining to the epidemiology of the disease, diagnostic modalities and general management strategies. 

Completing this survey will take you less than 10 minutes. By completing this survey, consent to have your data analyzed is implied. Participating in this study is completely voluntary and anonymous. You may choose not to participate. This survey is not sponsored by any pharmaceutical company. Upon completing the survey, you will be asked if you are willing to receive a summary and/or copy of the updated CTEPH guidelines and later participate in a post-guideline knowledge assessment survey.
 
This survey has been approved by the Canadian Thoracic Society CTEPH guideline committee.

The only potentially identifiable information, which is voluntary to enter, is the email address to allow us to distribute the follow-up survey. These email addresses will be kept in an encrypted, password-protected file. Once the follow-up survey has been completed, email addresses will be deleted from the data file.
