Exit Survey

Dear Artists, Performers, Booth Vendors, and Community,

We are administering a brief survey about the impact of our arts and culture-focused public space activations known as Created Commons, funded by the City of Seattle. In order to assess the quality and breadth of our event series' impact through our programming and logistics, we encourage you to fill these questions out to the best of your knowledge.

To further the goals of the City's Race and Social Justice Initiative, we collect demographic information that will be shared at the aggregate (group) level. For any questions about the survey, please contact Office of Arts & Culture at (206) 684-7171.  The feedback portal will remain open for 3 weeks. Thank you!

Sincerely,

Jenny Ku and the ARTS Impact and Assessment Team
