Introduction

Question Title

* 1. Informed Consent Form

We are conducting a survey to understand the reactions of individuals at home and at work to the COVID-19 crisis. We are documenting the impact on your finances, use of technology, psychological well-being, and creativity during this time period. We plan to disseminate our findings to increase awareness of the human experience, share present needs with policy makers, and learn how to help society better prepare for the future.

You will be asked questions, mostly short answer, and that may take 10-20 minutes to complete. It is best taken on a computer or tablet, not on your phone.

There are no risks to you. Your responses will help researchers study well-being and technology use during the crisis. All information is handled confidentially, and your responses will not be associated with your name in any way. No individual information will be reported; only mean level results will be reported in press releases or for research articles.

Your decision to allow us to use your data in this study is completely voluntary. You can choose not to participate, without negative consequences, by checking the box below.
