The ministry created this form for authorized court transcriptionists (ACTs) to report issues with a court recording that negatively impacts the ACT’s ability to produce a quality court transcript. 

When completing the feedback form, all questions below must be answered with as much detail/specificity as possible to enable court management to review the recording and identify the issue(s).  Only forms completed in detail will be investigated.  Please note, local management may contact you once they have reviewed the recording.
