Survey Instructions:

If you have not yet done so perhaps review the National Community Action Partnership’s Concept Paper before completing this survey. The paper provides background on COVID-19 case finding, contact tracing and resource coordination efforts and probes possible options for the CAA Network to collaborate with public health departments to ramp up testing and tracing efforts. Public health departments will be looking to expand their capacity in the coming weeks and CAAs may be in a unique position to provide assistance. Communities that identify and isolate contacts of those who test positive quickly will cut off the chain of transmission helping to get local economies open and staying open.

CAAs have a unique skill set and capacity that public health departments may find useful for contact tracing and resource coordination.

The National Community Action Partnership is collecting this information in order to get a better understanding of the scope of interest and potential related to COVID-19 contact tracing efforts and to determine what support might be necessary and helpful to the Community Action Network.

Thank you for taking the time to complete this survey. Please provide only one response per agency.
