What is this?
The Lyme Individual and Community Assessment (LICA) is a survey that asks about Lyme community members' needs and choices as we live our lives in Lyme.
 
Who do we want to answer this?
All of Lyme community members 18 years and older are asked to complete this 20 minute, confidential survey.
 
Why?
You will provide valuable information about Lyme residents
  • To get a current understanding of the health and well-being of  the community of Lyme;
  • To learn how best to serve and connect individuals in Lyme;
  • To help CommunityCare of Lyme and other organizations serving Lyme make good decisions about programs and resources for Lyme.
 
Section A asks for specific kinds of services and support you may need or be willing to help others with.  Sharing your name in this section WILL help to connect you with available services.  It WILL NOT be used to identify your info to anyone other than the persons managing the data.

This survey contains 70 questions and will take about 20 minutes to complete.
