We have been asked by the DHBs to find out what cognitive assessment tools are currently being used in general practice.
- In this survey, "cognitive assessment tool" refers to any standardised test that you conduct as part of an initial cognitive assessment process.

Why are we conducting this survey?
It is now one year since the recommended cognitive assessment tool for New Zealand General Practice changed from the MoCA© (Montreal Cognitive Assessment) to the Mini-ACE (Mini Addenbrookes Cognitive Examination). The DHBs would like feedback about the uptake of the Mini-ACE.
