Formal Review of the Student Code of Conduct (School Leaders)

Dear School Leader,

Thank you for your leadership during the initial implementation phase of the new Student Code of Conduct.

The purpose of this survey is to gather your feedback on the clarity, effectiveness, and practical applicability of the written Code document itself to inform necessary revisions and updates. This check-in marks the start of our formal review process.

Time Commitment: This survey should take approximately 10–15 minutes of focused time to complete.

Confidentiality: All responses are anonymous and confidential. Please be candid in your feedback.

Deadline: Please submit your responses by end of business on December 4, 2025.

Part 1: Overall Clarity & Effectiveness
1.The purpose and philosophy of the Code of Conduct are clearly articulated.
2.The language used in the Code is easy for all school staff to understand and apply consistently.
3.The tone of the document effectively promotes a positive, restorative approach rather than a punitive one.
4.The Code of Conduct is easy to communicate and explain to parents/guardians.
Part 2: Policy Clarity & Application
5.The four-level consequence system is clearly defined and provides enough guidance for staff decision-making.
6.The personal devices policy is clearly defined in the Code.
7.The Code clearly defines the roles and responsibilities of students, staff, and parents.
8.The Code adequately supports the integration of Restorative Practices as the primary response strategy.
Part 3: Conflicts & Gaps
9.Which specific rule, statement, or policy is the most challenging to implement consistently? Why?
10.Are there any 'grey areas' or conflicts in the Code that have led to confusion or disagreement among staff or with parents? Please explain
11.Does the Code adequately address student conduct outside of the school day (e.g., cyberbullying, transportation)? If not, what needs to be added?
12.From a practical perspective, what one specific change to the language of the Code would most improve your ability to lead your school?
Part 4: Proposed Revisions
