Why is Cochise Health & Social Services (CHSS) conducting a "Community Health Assessment?

We are conducting this survey to find out where we stand, as a community and county, on various health issues such as access to medical care, having a primary care doctor, etc. This survey is also part of a statewide project.

How long will it take to complete this survey?

About 10-15 minutes depending on your familiarity with surveys.

Are my answers confidential?

Yes, all answers are completely confidential. We are not asking for any personal information, such as your name and address, and we cannot tie answers to a specific person.

Will I be able to see the results of this survey?

Yes, we will post the "Cochise County Community Health Assessment” on our web site, early in 2013. You may request a copy by calling (520) 432-9400.

What will CHSS do with the results of this survey?

We will share the results with the community and work together to find solutions to our health challenges by developing and implementing a Community Health Improvement Plan. The Arizona Department of Health Services will compile an assessment for the State of Arizona including all counties.
