Employee Recognition Nomination Form

This program is intended to encourage City employees to go beyond our citizens' expectations and meet the highest standards of service to the community. Recognizing the useful role incentives play in encouraging superior employee performance and innovation, this program serves to reward performance and dedication that exceeds normal expectations.

This program is designed to reward City employees for exhibiting excellence in one of the City's four Core Values.

All employees are eligible for this program if they have been employed by the City for at least 6 months and exhibit the minimum criteria specified for each award.

At the end of each quarter, a committee selected by the City Manager will review the nominations and will award a winner for each Core Value area for which there was a nomination. All nominees will be recognized in the City Manager's Monthly Update and winners will receive a $25 gift card. Awards will be contingent upon the City's approved budget.

They strive to keep all promises and commitments. They insist on honesty and transparency in all dealings. They take responsibility for their individual actions.
