Dear Applicant:
On behalf of the College of Medical Laboratory Technologists of Ontario (CMLTO), I would like to thank you for your interest in joining the CMLTO Voluntary Roster of Registered Medical Laboratory Technicians.
The Voluntary Roster is a list of laboratory professionals who have chosen to apply to be recognized as having met skills and knowledge criteria set by CMLTO. Enrollment is voluntary for all medical laboratory technicians and assistants practising in Ontario. Acceptance to the Voluntary Roster entitles one to use the designation “Registered Medical Laboratory Technician.”
By applying for acceptance to the Voluntary Roster, applicants agree to meet the obligations outlined in the Code of Ethics and Standards of Practice for Registered Medical Laboratory Technicians, which are available at www.cmlto.com. Noncompliance with these obligations results in removal from the Voluntary Roster. The CMLTO reserves the right to amend the Voluntary Roster program in the future.
The information provided in this application will be used to determine your eligibility to be enrolled, based on meeting at least one of the following three (3) criteria:
1950 hours of general clinical laboratory and/or phlebotomy work experience over the last five (5) years.

Graduation from a medical laboratory technician or assistant program that is:
- Accredited by the Canadian Medical Association, or
- Approved by the Ontario Ministry of Training, Colleges and Universities, or
- Approved by the Ontario Society for Medical Technologists (OSMT).
OR
Current certification with the Canadian Society for Medical Laboratory Science (CSMLS) or the Ontario Society for Medical Technologists, and 900 hours of general clinical laboratory and/or phlebotomy work experience within the last three (3) years.
The Voluntary Roster Application fee is $113.00 ($100.00 + $13.00 HST). Upon acceptance of your application, you will be required to pay the Voluntary Roster fee of $56.50 ($50.00 + $6.50 HST).
Once CMLTO receives your completed online Voluntary Roster Application and supporting documents, we will begin to process your application. In most cases, applications are processed within 15 business days. If your application or supporting documents require additional assessment, processing may take up to eight weeks.
