4.Background
With new care transformation initiatives underway by the California Department of Health Care Services, including the new Medi-Cal Coverage of CHW/P/R Services, many providers and payers are working to identify and support CHW/P/R employers in preparing for operationalization.
With support from Health Net, HealthBegins invites all organizations that employ and/or recruit CHW/P/Rs to participate in this brief survey.
This survey aims to gather information about CHW/P/R employers' workforce count and existing services that may be reimbursable under the DHCS benefit. Preview the survey here.
The estimated time for completion is 10-15 minutes. This survey does not ask for any personally identifiable information about CHW/P/Rs.
The deadline to participate in the survey is Friday, May 19, 11:59 PM PT. All participants will be entered in three gift card raffles (one valued at $500 and two valued at $250 each). Participants will also be provided with results of the survey with aggregated data.
