Introduction

We are asking for your help in assessing our church. Church of the Palms UMC needs to understand in detail how you feel about your church life and the programs offered. We also would like to know where you want the church to go in the next three years.

Please take a few minutes to complete this survey. It should take less than 10 minutes to finish. If you are completing a paper survey, please place the completed survey in the church office. If you would like a link to the online survey, please let us know.

It is very important that everyone fill out a survey on paper or online by  April 30, 2018.  We want everyone 13 years old or older to be involved in this process and each person needs to fill out a survey independently. (Married couples and family members should each do an individual survey.)
