Chapters and volunteers are at the heart of the CPF Network; it is thanks to your efforts and commitment that we are able to make an impact.

To better assist you, we want to understand the issues and concerns you face in your volunteer work and the types of support that would be helpful to your Chapter.

Completing this survey will take between 10-15 minutes. Questions preceded by an * must be answered in order to move forward. Only one form should be completed per person, but multiple forms can be submitted by the same Chapter.

Please note that while the survey "closed" on Monday December 10, 2018, we still welcome feedback from Chapter representatives who didn't have the time to complete the survey before this deadline. We will continue collecting feedback in early 2019 and will be sure to account for your comments.
