New Chapter Application

Thank you for your interest in starting a new chapter of OEFFA!

There are three steps to start a new chapter:

Step 1: Fill out this application; return to OEFFA's Member Care Associate by submitting online, by email or mail

Step 2: Obtain three signatures (chapter leader and two others) on a written request letter to accompany your application; return these to OEFFA's Member Care Associate

Step 3: Receive Board approval to move forward

Let's get started with Step 1 by answering the questions below!
1.The new chapter facilitator has the primary responsibility for setting up the chapter in accordance with OEFFA policy and identifying participants. Please provide this individual's contact information below (a chapter president may be elected at a later date, as determined by chapter membership):(Required.)

9.Starting with the new chapter facilitator, please provide some background information for each chapter steering committee member known at the time of application, including any experience related to the proposed chapter).(Required.)
10.Including its chapter leader and steering committee, each new chapter must have a minimum six (6) members (representing different households) prior to the application being reviewed by the board. In the blanks below, please provide the contact information (including name, address, phone, email, and OEFFA membership status) for chapter members in addition to your steering committee. 11.The next several questions cover chapter functions you may or may not have started to think about. Just answer the questions to the best of your ability and when finished, rank your level of preparedness.
