CNHI Personal Phone Setup Request

1.Check here if this is your personal cell phone
2.Location number:(Required.)
3.Newspaper Name:(Required.)
4.Newspaper City:(Required.)
5.Newspaper State:(Required.)
6.Employee's name:(Required.)
7.Employee's cell phone number which email will be installed: (10 digits)(Required.)
8.Cell phone make: (i.e. iPhone, Samsung, LG etc.)(Required.)
9.Cell phone model: (i.e. Galaxy S6, iPhone 7s etc.)(Required.)
10.OS/IOS version: (i.e. i.E Android 6.0.1 (Marshmallow), iPhone 10.0.1 etc.)
On iPhone: Go to Settings > General > About > Version
On Android: Open web browser from phone and go to https://whatismyandroidversion.com
(Required.)
11.IMEI: Retrieve the IMEI/MEID number on virtually any cell phone by dialing in the universal code. Dial *#06# then hit call. (Required.)
12.Employee's email address: (the work address to be setup on the phone)(Required.)
13.Cell phone service provider: (i.e. AT&T, Verizon, Sprint etc.)(Required.)
14.Have you signed the CNHI Cell Phone policy and submitted it to your supervisor?(Required.)
Once this form is submitted you will be contacted by CNHI I.T. at the email address you provided above. 
A remote session will be necessary to ensure your workstation is setup correctly.
Once this is complete, you will receive information via text that will direct you to the app that will be installed on your cell phone.
