Want to Help Provide Vermonters with Telehealth Connectivity?

COVID-19 has heightened the need for telehealth services as an essential component of Vermont’s healthcare delivery system. The Connectivity Care Packages (CCP) Pilot Program promotes equitable access to telehealth services by getting equipment into the hands of Vermonters who need it and supporting patients with digital literacy skill development. This Pilot Program targets patients who have experienced a disruption of in-person care due to COVID-19, beginning March 1, 2020.

This extension of the CCP Program is being administered by Vermont Program for Quality in Health Care, Inc. (VPQHC) in partnership with All Clear EMG/Vermont Healthcare Emergency Preparedness Coalition (VHEPC) Please refer to www.vpqhc.org/ccptelehealthvt for more information.

When selecting CCP Pilot Program participants, VPQHC will prioritize organizations that serve vulnerable, low-income or otherwise underserved patients/residents in Vermont, as mandated by the federal grant. Selected medical practices, clinics, residential facilities and other health and wellness related organizations will receive free Apple iPads to distribute to their residents, patients, or clients for telehealth purposes, with user-orientation instructions and digital literacy resources. Wi-Fi signal boosters are also available. Pursuant to the terms of the federal grant, CCP is unable to fund any subscriptions to Wi-Fi, Broadband or cellular data service plans.

One of this Pilot Program’s objectives is to ascertain gaps in the access, deployment and adoption of telehealth services as a viable and effective option within the healthcare landscape in Vermont. To achieve this objective and recommend impactful solutions to these identified gaps, CCP requires that selected organizations perform basic data collection and equipment tracking activities. The intent is to fortify this Pilot Program toward actionable outcomes with minimal administrative burden on your end.
