"Journey to Stability" and what that means for Psoriasis patients

The Canadian Association of Psoriasis Patients (CAPP) and Canadian Psoriasis Network (CPN) are examining what “stability” or being “well controlled” means to Canadian patients living with moderate to severe Psoriasis and the journey they take to become “stable” or “well controlled”. The insights gained from this survey will help shape a national strategy and support future education and advocacy efforts.

This survey will take approximately 20 minutes to complete, depending on your responses.

Should you have any questions or technical difficulties, please contact executivedirector@cpn-rcp.com.

Although questions are written for individuals who are living with psoriasis, we welcome feedback from caregivers, friends or colleagues.

There is a mixture of multiple choice and open ended questions in the survey. The majority of multiple choice questions provide you with the opportunity to also provide additional comments. We encourage you to share all your thoughts and experiences as related to each question. YOU WILL NEED TO CLICK "NEXT" AT THE BOTTOM OF THE PAGE TO MOVE TO NEXT SECTION.

All information collected is confidential and results are aggregated.
We thank you in advance for participating.

NOTE: To qualify your name to be entered into a draw for a mini-iPad, you are required to fully complete the survey and provide your contact information at the end of the survey
