Thank you for Participating in the Anonymous Open Cannabis Consumer and Patient Survey led by Cannabis Community Care and Research Network (C3RN) and UMass Dartmouth!
 
Your input will be used to further the evidence around medical and adult-use cannabis. It will also be used develop free and accessible evidence-based education resources for cannabis patients and consumers. 

This information will be used to advocate for more cannabis research and funding to conduct rigorous cannabis research studies in Massachusetts and beyond. You can find study findings updated in real time here: www.cannacenterofexcellence.org/survey 

You are being asked to participate in a voluntary anonymous research study led by Cannabis Community Care and Research Network (C3RN) in collaboration with UMass Dartmouth. You will be able to see the results of your answers compared to others who have responded at the end of the survey! 
C3RN clinical study partners include Healer.com, Dr. Ryan Zaklin. C3RN has also partnered with registered medical cannabis dispensaries in Massachusetts INSA in Easthampton and Springfield, MA, Revolutionary Clinics in Somerville and Cambridge, MA and BASK in Fairhaven, MA to conduct the survey. 

This form provides you with information about the study.  If you have questions about this study please email coordinator@c3researchnetwork.com. Your participation is entirely voluntary.  You can refuse to participate without penalty or loss of benefits to which you are otherwise entitled.  You can stop your participation at any time and your refusal will not impact current or future relationships with UMass Dartmouth or C3RN.  You may skip any questions that you are not comfortable answering.  To discontinue participation you should close your internet browser window. The purpose of this study is to determine consumer and patient preferences, consumption methods, information needs, access, and benefits associated in 30 states with legal recreational and medical cannabis.  The collection of this information is intended to be published in peer-reviewed scientific literature and used to create evidence-based cannabis education resources for consumers and patients. 

If you agree to be in this study, we will ask you to do the following things:  Complete a 60+-question survey

Total estimated time to participate in the study is less than 30 minutes. WE THANK YOU for your time and input! 

Risks of being in the study: The risks of this study are no greater than those associated with everyday life.

Benefits of being in the study: Your participation helps to build the body of knowledge regarding medical cannabis patient and consumer preferences, benefits, information needs, stigma, and access to cannabis.

Compensation
You have several options for compensation for your time in filling out this survey. At the end you will have the option to select any or all of the following: 
Confidentiality and Privacy Protections 
We have taken all reasonable measures to protect your identity and responses.  The questions in this survey do not ask you to reveal any personally identifying information, the transmitted data are SSL encrypted and stored in a secure password protected database, and IP addresses are not collected.  However, email and the internet are not 100% secure, so it is also suggested that you clear the computer’s cache and browser history to protect your privacy after completing the survey.  The data resulting from your participation may be made available to other researchers in the future for research purposes not detailed within this consent form. In these cases, the data will contain no identifying information that could associate you with it, or with your participation in any study.

The records of this study will be stored securely and kept confidential.  Authorized persons from The University of Massachusetts Dartmouth, members of the Institutional Review Board, and Cannabis Community Care and Research Network have the legal right to review your research records and will protect the confidentiality of those records to the extent permitted by law.  All publications will exclude any information that will make it possible to identify you as a subject. Throughout the study, the researchers will notify you of new information that may become available and that might affect your decision to remain in the study.
