Privacy and Confidentiality
The responses to this questionnaire will be treated in confidence and anonymity of respondents and their business is ensured. The questionnaire is carried out in accordance with the Data Protection Act. The questionnaire may look lengthy but it should only take around 15 minutes to complete.
The questionnaire has four sections and should be completed by a senior person on behalf of the business.
