The DPH Bureau of Substance Addiction Services (BSAS) is interested in substance use/addiction workers and how they make job changes.

We hope to find out more about workers' experiences to help inform BSAS on how to better retain workers in the field. We are asking you to complete this optional survey because you have recently left or are leaving your job. We are not asking for your name (the survey is anonymous).
