1. Lifetime Achievement Nomination Form

Nominees are currently being sought for the prestigious Arts For Life Lifetime Achievement Award. This award will be presented to the recipient at the Arts For Life Best Performance Awards & the Theatre Mask Awards Events held annually.

Eligible candidates must be a living contributor in the arts and have:

1. Contributed significantly to the Arts
2. Been born in the Metro-St. Louis Area or
Spent formative/creative years in the Metro-St. Louis Area
3. Had accomplishments/contributions that have had local, national, or international impact on our cultural heritage.

To nominate an individual for this award please fill out the below questions. All submissions will be reviewed by the ARTS FOR LIFE Board of Directors.
