INFORMED CONSENT FORM

Title of the Study: Insights into Body Image in School Curriculum in Canada: Perspectives to Inform Future Directions

Principal Investigator: Dr. Catherine Sabiston (catherine.sabiston@utoronto.ca), Professor, Faculty of Kinesiology and Physical Education, University of Toronto.

Introduction:
The purpose of this brief survey (<5 min) is to gather perspectives on the current state and future of body image in school curriculum across Canada. This input will help inform ongoing research, and knowledge mobilization efforts related to body image, youth well-being, and education.

Eligibility Criteria:
You are eligible to participate if you are:
18 years of age or older and
have experience or interest in youth body image education
educator, researcher, health practitioner, school administrator

Study Procedures:
Participation involves completing a short anonymous survey that will take less than 5 minutes. You will be asked a small number of questions about your perspectives on body image in school curriculum.
Compensation: There is no compensation for participation in this survey.

Potential Benefits:
There are no direct personal benefits. However, your responses will contribute to research and discussions aimed at improving how body image is addressed in school curriculum across Canada.

Potential Risks:
This study involves minimal risk. Some questions may relate to body image, which may be a sensitive topic. You may skip any questions or stop participating at any time.

Confidentiality:
No identifying information (such as name or email) will be collected. All responses are anonymous and will be reported in aggregate form. Data will be securely stored at the University of Toronto and retained for research purposes.

Voluntary Participation and/or Withdrawal:
Your participation is entirely voluntary. You may choose not to participate or to stop the survey at any time without penalty.

Dissemination and Debrief
Findings from this survey may be used in presentations, reports, and academic publications. Results will be presented in aggregate form and will not identify any individual participants.
