Nominate a Colleague for The 2024 Martin L. Block Award
The Dr. Martin L. Block Award is presented each year to an individual who has made a significant impact in the lives of America’s seniors through clinical leadership, policy vision, and through superior example of the RISE mission to promote continuous improvement in the health care system.
Please see the criteria below for submitting a nomination:
1) Nominee must work within the clinical field as an MD, NP, RN, PA, DO or otherwise clinically credentialed
2) Nominee should work for a Health Plan, Provider Organization, Accountable Care Organization or Vendor/Supplier in the health care marketplace
3) Nominee must have fulfilled one of the following criteria: contributed to significant process improvement in the care of seniors and/or dedicated 10+ years to exemplary work in this industry

This form must be filled out completely with necessary documentation in order for the nominee to be considered for the 2024 Dr. Martin L. Block Award. Nominations must be submitted by December 31, 2023. The award recipient will be presented at RISE National 2024.
