2. Demographic Survey

Instructions: Please answer the following questions as honestly as possible according to what best matches your perspective and/or lived reality.  There are no right or wrong answers.  The information from the demographic survey will allow the researchers to accurately describe the participants as a group.  It will also allow us to analyze what if any impact specific factors such as type of institution attended or gender identity have on responses.  All surveys will be completed anonymously; however, you may leave any question blank if you prefer not to answer it without penalty. All data will be evaluated as a group. Only randomly generated participant codes will be used to link demographic surveys to other data collection surveys.
