Family Discipleship Survey

Thank you for participating in this survey. Your feedback is important as we begin to diagnose and develop spiritual growth needs in our church families. 

This survey is intended for parents with children living at home. If your children are too young to participate in an activity that is described, please honestly assess what you anticipate doing when your children become old enough to participate. If your children are grown and no longer in the home, please select what best describes your experience while your children were living at home. 

For the purposes of this survey, "church leaders" includes pastors, ministers, deacons, teachers, or small group leaders.
