This is Part 2 of the online registration process for the BACON Teen Mission trip to Honduras on August 7-16 in La, Ceiba.  If you have not filled out Part 1 yet of the online registration process, please go here. (Both parts are required!)

The purpose of Part 2 is to obtain some more info about you, and also for you to indicate your commitment to the trip, and what this means.

You will be officially registered for the trip to Honduras once you have completed Parts 1 & 2 of online registration, as well as pay a $200 deposit.  All of this should be done by Sunday, March 25th.  We will select the team the final week of March, and then communicate with each applicant to let them know if they are selected.  If you are not selected for the team, then you will be refunded your deposit.

You can find more details about the trip by downloading the information packet distributed at the March 4th trip meeting.  This packet can be downloaded from our teen ministry webpage.

COMMITMENT on your part is very important for this mission trip to Honduras.  Please answer the following statements of commitment below.  If you must answer "no" to any of them,  please indicate the reason in the comment box at the end of the section.

* 5. I will read all handouts distributed about the trip and check my email regularly to keep up with all communication about the trip, and respond as requested to questions and requests in those communications.

* 6. I will give my full effort while on the trip in Honduras and respect all leadership and fellow team members.

* 7. I will be able to attend the team meeting on Sunday, April 29th, at 12:15pm.

* 8. I and my parents will be able to attend the team meeting on Sunday, June 3rd, at 2:00pm.

* 9. I will be able to attend the team meeting on Sunday, July 22nd, at 2:00pm.

* 10. I will be able to attend the team meeting on Sunday, August 5th, at 2:00pm.

* 11. I and my family will be able to assist with the Easter Sunday Morning Breakfast on Sunday, April 1st.

* 12. I and my family will be able to help with an all-church fundraising dinner on Saturday night, June 2nd, highlighting our mission trip

* 13. I will make every effort to be fully involved in other team fundraising that is planned.

* 14. I understand that I will need to obtain a passport for this trip to Honduras.  If I have a passport that will expire this year, then I will need to renew my passport before going on the trip.

* 15. I will follow the posted payment schedule for the trip, or communicate with Pastor Gregg about any alterations to the schedule.  If paying for the trip will be a challenge, I commit to being in prayer that God will provide, and will ask Pastor Gregg to join me in that prayer.

* 16. I will make sure I have an updated tetanus shot for the trip.

* 17. (If this is a teen) For the parent/guardian.  My son/daughter has discussed going on the mission trip to Honduras and I support him/her going on the trip.
