Dear delegates and friends,

Your wellbeing is our highest priority and we are committed to working with our event partners to provide a safe and hospitable environment at the AVPN Southeast Asia Social Investment Summit 2020 in Bali, Indonesia.

Please fill in the form to inform us on whether you have travelled to China in the last 14 days.

We would like to inform all delegates planning to attend the AVPN Southeast Asia Summit, that if you have been in China in the last 14 days prior the Summit OR have had close contact with another person who has been in China in the last 14 days prior the Summit, we must regretfully advise you to not join us in Bali this year, in the interest of your health and that of the Summit delegates and partners.

We regret the need to implement these restrictions, and respectfully hope for everyone's understanding of our aim to take prudent measures to ensure the health and safety for our members and partners in all markets. Should you need to cancel your attendance due to this restriction applying to you, kindly get in touch with us for processing the cancellation of your summit ticket.

Indonesia is temporarily banning flights to and from mainland China from Feb. 3, 2020 and will not allow those who have been there in recent weeks to enter or transit. The temporary flight ban applies to foreign airlines flying from China to Indonesia, including for transit.

This form is for the purpose of obtaining the travel history of delegates and will not be used for any other purposes.
