The Department of Aging and Disability Services (DADS) is requesting your input on this survey regarding Comprehensive Assessment Instruments for Individuals with an Intellectual or Developmental Disability (IDD). It should take less than ten minutes to complete.

In accordance with Senate Bill 7 (83rd Legislature, 2013) DADS shall evaluate comprehensive assessment instruments for individuals with an IDD as needed to ensure that each individual with an IDD receives the type, intensity and range of services that are both appropriate and available, based on the functional needs of that individual.

The survey asks you to assess 9 criteria by ranking them in order of importance (1 = most important and 9 = least important).
